Provide home-based care to patients
with a prognosis of six months or less to live, focused on quality
of lifeWork as part of an interdisciplinary team comprised of a
physician, RN, CNA, HHA, social worker, chaplain and volunteers to
carry out the patient's plan of carePerform nursing services which
are planned, delegated, and supervised by the registered
